How the signup process works

1. Complete the sign-up questionnaire online
2. Print, sign, and fax the contract(s) generated at the end of the questionnaire
3. Upon review and acceptance, you will receive a welcome email with all the pertinent details and further instructions

You may find it helpful to have:

• Drivers License and Social Security Numbers
• Issue date, expiration date, and license number for each state you are licensed
